Job Title: Physician/Nurse Practitioner - Health Risk Assessments

Job Overview

Medigence Health is seeking dedicated Physicians and Nurse Practitioners to conduct Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace beneficiaries. This role offers a unique opportunity to engage with patients in various settings, including home visits, clinics, telemedicine, and long-term care facilities. As a key member of our clinical team, you will play a vital role in enhancing the quality of life for the individuals we serve.

Key Responsibilities

• Conduct comprehensive evaluations of medical and social needs using Medigence Health's tablet-based assessment tool.
• Identify and facilitate referrals for members requiring routine, urgent, or emergent case management.
• Provide recommendations for follow-up with primary care providers.
• Travel across local counties and regions where you hold a valid license, primarily within Wisconsin.
• Engage with members in their homes or through telehealth platforms.
• Collaborate with healthcare teams to ensure comprehensive patient care.
  
  

Required Skills

• Minimum of 2 years of relevant experience as a Nurse Practitioner or Physician.
• Familiarity with HCC model, HEDIS, and STAR quality measures.
• Current, unrestricted state license or ability to obtain one.
• Registration with Medicare and Wisconsin Medicaid is mandatory.
• Board certification is preferred in internal medicine, geriatrics, or family medicine.
• Strong communication skills, particularly with elderly and chronically ill patients.
• Proficiency in utilizing computer, tablet, and mobile technologies.
  
  

Qualifications

• Relevant degree in Nursing or Medicine from an accredited institution.
• Board certification in an applicable specialty is preferred.
• Experience working in home visit environments or utilizing telehealth technology is advantageous.
