About The Role
As the PACE Physical Therapist, you will play a crucial role in our interdisciplinary team at Seen Health, providing comprehensive and coordinated care to elderly participants in the Program of All-Inclusive Care for the Elderly (PACE) in San Gabriel Valley. Your focus will be on assessing, planning, implementing, and evaluating physical therapy interventions to enhance participant mobility, functionality, and overall quality of life.

About The Role
As the PACE Physical Therapist, you will play a crucial role in our interdisciplinary team at Seen Health, providing comprehensive and coordinated care to elderly participants in the Program of All-Inclusive Care for the Elderly (PACE) in San Gabriel Valley. Your focus will be on assessing, planning, implementing, and evaluating physical therapy interventions to enhance participant mobility, functionality, and overall quality of life.
Responsibilities
- Conduct comprehensive physical therapy assessments to evaluate participants' functional abilities, mobility, and safety in their living environments.
- Collaborate with the Interdisciplinary Team (IDT) to develop individualized care plans that address participants' physical therapy needs and goals.
- Provide direct physical therapy interventions based on established care plans, including therapeutic exercises, gait training, balance exercises, and mobility aids recommendations.
- Instruct participants, families, and caregivers on proper techniques for exercises, mobility, and fall prevention strategies.
- Supervise and educate Personal Care Assistants and other team members on safe transfer techniques, positioning, and assistance with mobility tasks.
- Maintain accurate and timely documentation of participant progress, interventions, and outcomes in electronic medical records.
- Coordinate with external service providers, such as durable medical equipment suppliers, to ensure seamless continuity of care for participants.
- Participate actively in IDT meetings to discuss participant progress, review care plans, and make recommendations for adjustments as needed.
- Adhere to center policies and procedures, maintain confidentiality of participant information, and comply with regulatory requirements.
- Graduate of an accredited physical therapy program; Doctor of Physical Therapy (DPT) required.
- Valid and active California Physical Therapy license.
- Proficiency in Mandarin and/or Cantonese preferred but not required.
- Minimum of 2 year of experience in physical therapy, preferably in geriatric or community-based care settings.
- Strong communication, interpersonal, and organizational skills.
- Ability to work effectively in a team-based care environment and collaborate with multidisciplinary healthcare professionals.
- Los Angeles required. Ability to commute to Alhambra required.
