Works collaboratively with the Primary Care Provider team to identify and address clients’ barriers to achieving high levels of health (health care, child care, housing and social services) for Partnership Health Center (PHC).

Representative Examples of Work

Works to address disparities in vaccination and testing, and works to improve access to health care, housing, child care and social services to improve population health outcomes.

Conducts an assessment of clients’ needs using a variety of screening tools to identify issues such as mobility problems, depression, and needs related to the social determinants of health, etc. Connects patients with appropriate clinical staff when there are questions about their treatment plans or health care recommendations.

Identifies, anticipates and assists clients with community, cultural, social and educational barriers within the healthcare system. Assists patients in removing barriers to care and individual self-management goals by encouraging self-sufficiency and advocacy. Models appropriate strategies for self-help and coping skills with effective communication.

Provides peer health education through home visits and/or clinic visits to assess clients’ needs and concerns related to their health, family, and community. Provides educational materials for patients. Documents interventions and maintains accurate records.

Builds and maintains relationships with clients, support networks and community agencies. Provides client advocacy. Refers clients appropriately to resources such as transportation, housing, food security or financial assistance. May coordinate and/or attend appointments with clients.

Participates in community outreach activities as directed to connect clients to services, including outreach to the unsheltered population. Prepares and presents educational material for the community.

Participates in scheduled staff development trainings to increase understanding of the various chronic disease, effective team functions, and trauma-informed care. Regularly meets with program coordinator to discuss cases and perform effective management and conferencing.

Performs related work as required or directed.

SUPERVISION RECEIVED: Works under the close supervision of the Program Development Manager.

SUPERVISION EXERCISED: None.

WORKING RELATIONSHIPS: Has numerous contacts with health care providers, community health specialist, program manager, behavioral health care manager, clients, co-workers, and a wide variety of outside agency representatives to provide cohesive and coordinated efforts for improved health and well-being for individuals and the community as a whole.

Required Knowledge, Skills & Abilities

KNOWLEDGE: Working knowledge of PHC clinical operations. Working knowledge of community resources. Working knowledge of the PHC Corporate Compliance Guidelines. Basic knowledge of the impact of trauma-informed care and adverse childhood experiences (ACES). Basic knowledge of client confidentiality, and the ethical and legal requirements of working with private medical information. Basic knowledge of customer service. Basic knowledge of the principles and practices of HIPAA compliance. Basic knowledge of the principles and practices of clinical assessments and health care management. Basic knowledge of medical terminology.

SKILLS: Skill in the use of a personal computer and related software (Microsoft Office).

ABILITIES: Ability to recognize social and medical risk factors, trauma symptoms, behaviors, and chronic disease conditions. Ability to work as part of a team. Ability to motivate, engage and work collaboratively with others. Ability to remain calm and poised in urgent situations. Ability to communicate effectively verbally and in writing. Ability to support PHC’s mission and provide culturally sensitive care. Ability to prioritize multiple duties and respond to changing circumstances. Ability to solve problems, deal with issues and concerns in a positive and constructive manner. Ability to use initiative, good judgment, and receive feedback. Ability to establish and maintain effective working relationships with diverse individuals and groups, particularly when dealing with sensitive and issues. Ability to work a varying schedule.

Minimum Qualifications

An equivalent combination of education and experience may be considered.

EDUCATION: High school graduation or GED.

EXPERIENCE: Requires one year of experience navigating social services, counseling, health/behavioral healthcare, health education, and working with marginalized populations, Indigenous communities, and/or economically disadvantaged populations. Lived experience with at least one social determinant of health such as chronic disease, homelessness, and economic insecurity desired.

SPECIAL REQUIREMENTS : Requires immunizations or proof of immunity to certain infectious diseases and a TB test. Requires a valid Montana Driver's License. Completion of PHC Community Health Worker Training within 6 months of hire required. New employees will be asked to volunteer vaccination status for required vaccines upon hire and will be offered assistance during hiring to receive necessary immunizations. Employees who have not received the vaccines required for their positions or who are unwilling to voluntarily provide vaccination status for required vaccines will receive a reasonable accommodation where such accommodation does not require an undue hardship or endanger the health or safely of any person.

Physical/Environmental Demands

The work is performed in the PHC clinic and office settings, outreach events, patient homes, and other community settings and may expose the employee to communicable diseases. Requires bending and medium-range lifting (up to 30 pounds). May require attending meetings outside of normal work hours. May meet unsheltered clients outside in inclement and changing weather conditions. May require walking over uneven terrain and climbing stairs. This position is not eligible for remote work.

Excellent Medical Health Benefits– Group health benefits, including dental and vision coverage, are available to eligible County employees. The medical health benefits premium is covered for full-time employees. The County contribution is pro-rated on hours paid for part-time employees. The employee pays the premium portion for dependent coverage. Family health benefits coverage is $258.00 per pay period.

Medical deductible is $500 for an individual and $1,000 for a family.

Flexible Benefits Plan– Pre-tax out-of-pocket medical expenses and day care expenses.

Public Employees Retirement System– Montana public employees of the state, university system, local governments and certain employees of school districts are covered by the Public Employees Retirement System (PERS). New members to the PERS have an opportunity to choose between two retirement plan options: the Defined Benefit Plan or the Defined Contribution Plan.

Sheriffs' Retirement System (SRS)– A public pension plan for all Montana Sheriffs hired after July 1, 1974 and Detention Officers hired after July 1, 2005.

Supplemental Retirement Benefitsare offered through Valic or Nationwide.

Sick Leave– Full-time employees accrue 7.38hours per month and are eligible to use sick leave once you have been an employee for 90 days.The accrual is pro-rated for part-time employees.

Vacation Leave– Full-time employees accrue 9.24 hours per month and are eligible to use leave after continuous employment for a period of 6 full months. The accrual is pro-rated for part-time employees.

Holidays– The County observes eleven legal holidays in even numbered years and ten legal holidays in odd numbered years.

Paid Parental Leave (PPL)-In recognition of the importance of bonding and care of a newborn child or a child placed for adoption, Missoula County 6 continuous weeks of PPL to eligible full-time employees that have been with the county for 180 days. The hours are pro-rated for part-time employees.
