Description

Bilingual in Spanish/English strongly preferred.

This position will require traveling to see patients in Napa County during the weekdays on day shift.

The Community Health Worker helps Latino individuals and their families, as well as identified local agencies serving the Latino community, better understand, access and utilize Collabria Care programs and services (inclusive of Hospice, Adult Day Health and Palliative Care). This role will perform assessments, develop care plans and participate in IDT. In addition this position will participate in community-based outreach activities in an effort to promote awareness, reduce barriers, identify educational opportunities, and support access to services.

Required Qualifications

  • Within 30 days of hire: National Provider BLS - American Heart Association

Preferred Qualifications

  • Associate's Degree Health or Human Services related field.
  • 1 year Health education experience with Spanish speaking population.

About The Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
