Remote Personal Care Coordinator | WFH Opportunity

Job Overview

We are pleased to announce an opening for the TEMP - Personal Care Coordinator position, designed for motivated and skilled individuals eager to contribute to our team dedicated to implementing the Health's Whole Child Model (WCM) procedures. This remote opportunity allows you to play a pivotal role in facilitating member referrals to the Orange County Health Care Agency (OCHCA) for cases that qualify under California Children Services (CCS). You will also be instrumental in managing Annual Medical Reviews (AMR) and Inter-County Transfers (ICT) for WCM members.

Key Responsibilities

• Assist in the processing of standard WCM referrals, AMRs, and ICTs in a timely manner.
• Collaborate with health networks to ensure prompt acquisition of necessary clinical data.
• Coordinate with OCHCA CCS to fulfill health networks’ clinical requirements.
• Work alongside Case Management professionals and other healthcare team members to streamline medical care and resolve member inquiries.
• Conduct outreach to members, supporting the completion of initial and annual Health Needs Assessments (HNAs) or Health Risk Assessments (HRAs).
• Communicate initial care plans to primary care providers, health networks, and members as required.
• Facilitate referrals for behavioral health services and assist members in accessing relevant community resources.
• Guide members in comprehending and utilizing their Medi-Cal and CCS benefits.
• Notify the care team of significant event triggers affecting member care.

Qualifications

• A High School diploma or equivalent is required.
• A minimum of 1 year of experience in a member service role focusing on pediatric needs is essential.
• Equivalent combinations of education and experience demonstrating the ability to fulfill the essential duties of this position may also be considered.
