Essential Functions (What You Do).
Pay: $22.50 to $36.50

Requirements
Education:
Master's Degree in Health Educationfrom a CHES accredited program.
License/ Certifications
Certified Health Education Specialist (CHES)
Basic Life Support (BLS) from a AHA approved program
Knowledge/Skills/Abilities
Familiar with the culture and customs of the clients with whom they will interact.
Communication skills consistent with the facilitation of health and wellness topics to individuals, small and large groups of community residents.
Demonstrates the ability to read, write and speak English and Spanish without an interpreter.
Demonstrates interpersonal/cultural sensitivity and work with a diverse and often underserved population.
Effective written and verbal communication across all levels of the organization and with vendors.
Highly organized with strong time management skills, being able to perform simultaneous tasks and prioritize tasks effectively and follow through in a timely manner.
Proficiency with EMR systems.
Salary Description
$25.000 to $36.50 per hour

- St. Jude Neighborhood Health Centers utilizes EPIC for our Electronic Medical Records (EMR) system.
- Part-time grant funded position working 24 hours per week (benefit eligible).
Essential Functions (What You Do).
- Provides direct patient education, using up-to-date, evidence-based guidelines and innovative practices in classroom, family and one-on-one settings.
- Successfully recruit patients into education initiatives; track patient engagement and outcomes.
- Serves as the Comprehensive Perinatal Services Program provider, delivering comprehensive perinatal education to patients in collaboration with our prenatal providers, facilitate appropriate referrals, and maintain documentation compliant with California regulations. Submit accurate and timely charges for all CPSP services. Support annual CPSP audit and program provider application updates.
- Enroll and maintain health center sites as CPSP provider sites. Coach and guide other CPSP educators on the Women's Health Team.
- Collaborate with care team members in achievement of strategic quality goals through development and deployment of standard workflows and strategic care interventions identified by our clinical leadership team.
- Participate in and support the delivery of group medical visits.
- Support providers and broader care team by helping to track, engage, and coordinate needed care for high-risk women's health / OB patients.
- Documents all patient related contact accurately and timely within each patient's medical record.
- Works collaboratively with all caregivers in an effort to deliver the highest- quality care to all patients. Consults with and supports the work of St. Jude Neighborhood Health Centers' providers in an effort to help patients meet their health goals.
- Travel to health center sites in Fullerton, Anaheim and Orange to provide patient education.
Pay: $22.50 to $36.50

Requirements
