JOB PURPOSE:
Ensures delivery of services to families and youth by linking them to crisis pregnancy services, foster care services, and or other community services. This program services 9 Texas counties: Austin, Brazoria, Chambers, Fort Bend, Galveston, Harris, Liberty, Montgomery, and Waller counties. This position supports rural geographical regions and travel is involved. This is a grant-funded program.
ESSENTIAL JOB RESPONSIBILITIES:
- Develops effective, trusting relationships with program participants by utilizing trauma-informed care and a strength-based approach. Models positive interactions, verbal guidance, and effective communication.
- Develops and maintains comprehensive outreach/working relationships with Clients and community partners on the participants behalf to develop workable plans to obtain and/or enroll in community services.
- Reviews clients needs assessment, connects clients to services in the community, and tracks outcomes.
- Follows care follow-up procedures to ensure families receive the services they were referred to.
- Assists in conducting outreach and networking activities in the community. Provides presentations at various community groups/events. Is active and networks in monthly community meetings. Promotes the success and reputation of the Houston FYSS program.
- Other duties as assigned.

ESSENTIAL QUALIFICATIONS:
EDUCATION: Bachelors Degree
EXPERIENCE: 1+ years of experience in a social service agency, 1+ years in a customer service-focused environment. Experience with mental health and/or a homeless population is a plus.
ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employers sole discretion.
LICENSES: Drivers License with clear record.
VEHICLE: Must have daily use of a vehicle without prior notice.
OTHER: Bilingual Spanish/English preferred. Must be available and willing to travel to various locations and at the frequency that the business needs dictate.
