Summary:

The Clinical Manager of Integrated Care will work under the supervision of the Chief Integration Officer as part of the Management and Clinical Services Team. This position is responsible for providing clinical and administrative leadership to the Adult Mental Health Case Management team which may include additional assigned clinical programs. The Clinical Manager is responsible for implementing client services that address Zepf Center mission and are provided in a manner consistent with the organization’s vision and values.


Essential Duties and Responsibilities:

  • Monitors provision of Adult Mental Health Case Management services and other assigned clinical programs. This includes ensuring the programs fully meet and exceeds Fidelity and the ODMH and CARF requirements.
  • Oversees all care coordination areas of comprehensive care management, care coordination, health promotion, comprehensive transitional care and follow up, individual and family supports and referrals to community support services.
  • Makes and receives referrals, processes case assignments, assures staff assigned to clients are delivering high quality services.
  • Provides direct supervision to assigned Case Coordination teams at various Zepf locations if necessary
  • Oversees orientation and training of new employees and ensures all appropriate paperwork pertaining to onboarding is complete and submitted on a timely basis.
  • Works in conjunction with the Chief to determine, establish, and maintain appropriate staffing levels to ensure adequate coverage of the facility and program.
  • Develops agenda and conducts team meetings.
  • Be eligible to become a Health Officer and provide risk assessments in community
  • Manage the day to day operations of assigned programs including budget formation and maintenance, purchase requisitions, and adherence to contracts and formal agreements.
  • Responsible for developing, implementing, evaluating, and enhancing Adult Mental Health Case Management services and other assigned clinical programs to meet the needs of consumers, families, funding sources, and other interested parties.
  • Builds effective relationships with community partners that enhances the overall health of clients enrolled in the assigned programs. Makes and receives referrals, processes case assignments, transfers, terminations, and assures staff assigned to clients are delivering high quality services.
  • Monitors and facilitates consumer identification and engagement process and completion of comprehensive health and risk assessments.
  • Oversees the team in Development and monitoring of person-centered care plans and progress towards outcomes.
  • Acts as agency representative of Adult Mental Health Care Management and other assigned clinical programs to the broader health care community including Medicaid Managed Care Plans, hospitals, and all other service organizations.
  • Fosters program development through ongoing marketing initiative.
  • Coordinates or facilitates hand offs as face to face interaction between providers to exchange information and facilitate Case Consultation between members of the treatment team.
  • May provide individual, group and family prevention and care coordination services both in agency facilities and in the community based on client.
  • Compliance with all documentation and productivity standards, certifications, and licensing standards.
  • Provide on-call supervisory support and on-site supervisory coverage for the Client Services Division as required. Works with the team members in a care management capacity, including coordinating and collaborating with the team members, psychiatrist, nursing, and other healthcare providers regarding services.
  • Perform other duties as assigned.

Requirements:
  • Master’s degree from an accredited college or university in social work, psychology, or other closely related human service field is preferred.
  • Ohio LSW, LPC or RN license required. Preference given to candidates with an Ohio LISW, LPCC license
