The Community Health Worker engages in a collaborative process, which assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet a patient’s community, behavioral, and clinical health needs through communication and available resources to promote quality, cost-effective outcomes.

The Community Health Worker collaborates with clinical teams in various healthcare settings in support of the patient, to include acute care, skilled nursing care setting, long term acute care setting, rehab facilities, custodial care, ambulatory care settings, and the patient’s home. The Community Health Worker serves as a liaison between patients or families/caregivers and the community/healthcare resources. This position will facilitate the coordination of care resulting in more efficient utilization of healthcare resources, decreased hospital readmissions and improved health outcomes.

This is a Field-Based Role and will require travel within/throughout the Greater Baton Rouge or Greater New Orleans, LA Market. Expect 50-75% Travel in those areas.

Primary Responsibilities:

Care Coordination and Patient Support:

  • Engages patient primary face to face or over the phone to discuss their health
  • Create a positive experience and relationship with the patient
  • Proactively engage the patient to manage their own health and healthcare
  • Support the patient to improve their well-being by staying out of the hospital and attend regular visit to their primary physician, via education, clinical resources, and/or patient follow-up
  • Support the member to ensure pick-up of their prescriptions by providing education, quality compliance, and patient follow-up
  • Provide member education on community resources and benefits
  • Listen actively, communicate with empathy and gather information in a respectful manner
  • Conduct outreach to encourage participation in health-related programs, services, and/or Providers
  • Use methods that promote learning and positive behavior change
  • Use a variety of interactive teaching and coaching methods for different learning styles
  • Prepare and distribute education materials and present or mail to patient
  • Performs all other related duties as assigned

Clinic Team Communication:

  • Participates in daily/weekly/monthly discussions where patients’ goals, outcomes, interventions, and recommendations for improvement are discussed with members of the patient’s care team

Required Qualifications:

  • High School Diploma/GED
  • 3+ years of experience in the delivery of services to assist with patient and/or families to address multiple issues in clinic, home, and/or community-based environment
  • 3+ years of experience in the community area
  • Computer skills requiring data entry and retrieval of patient information. Proficient with Windows-style applications

Preferred Qualifications:

  • Current Certification as Community Health Worker by the State of Louisiana
  • Current CPR certification
  • Experience with Electronic Medical Record

Physical Demands/Working Conditions:

  • Ability to travel by personal vehicle between various hospitals, clinics, and patient’s residence on a routine basis
  • Must be available to work extended hours, overtime and non-traditional hours as needed
