- Evaluate and treat patients in their homes, requiring travel to patient locations.
- Assess functional levels and develop individualized care plans.
- Evaluate home environments and provide appropriate recommendations.
- Collaborate with patients and families to achieve optimal outcomes.
- Los Angeles County: Los Angeles, Hollywood, Beverly Hills, Van Nuys, Simi Valley, Northridge, Thousand Oaks, Santa Monica, Culver City, Inglewood, Southgate, Gardena, Torrance, Hawthorne, Long Beach, East LA, Downtown LA, South LA, Glendale, Eagle Rock, Pasadena, Alhambra, Arcadia, Burbank, Diamond Bar, West Covina.
- Riverside County: Riverside, Moreno Valley, Temecula, Corona.
- San Bernardino County: Rancho Cucamonga, Chino, Rialto, Colton.
- Orange County: Huntington Beach, Santa Ana, Anaheim, Newport Beach.
- Current California Physical Therapy (PT) License.
- Valid California Driver’s License with reliable transportation and insurance.
- Full-time, Part-time, Per-Diem
