- Provide assistance to patients and families by screening for social determinants of health
- Identify opportunities to improve health
- Connect families to appropriate resources
- Follow-up to confirm the connections were made and beneficial
- Assist families in enrolling children into Medicaid, Children's Health Insurance Programs), and other public benefits.
