Social Worker responsibilities Include:
- Provide individual counseling and group therapy to residents and their families
- Assess the psychosocial needs of residents and develop care plans to address those needs
- Advocate for residents and ensure their rights are protected within the facility
- Assist with discharge planning, community referrals, and resources
- Collaborate with nursing, medical, and other interdisciplinary team members to ensure holistic care
- Must hold a current Social Worker license in New Jersey (LCSW, LMSW, or LSW)
- Previous experience in long-term care or healthcare settings preferred
- Excellent communication, interpersonal, and organizational skills
- Compassionate, patient, and committed to improving the quality of life for residents
