Promise Care NJ is seeking an energetic, caring, and professional Physical Therapist to join our Hudson or Essex County Home Health Care team.

Physical Therapist (PT) Responsibilities:

• Perform home visits to clients
• Perform diagnostic tests and measurements
• Develop and implement individualized care plans
• Perform continual assessments
• Coordinate with other members of patient's care team
• Oversee PTA functions.
• Provide education and hands-on instruction to patients, their care takers, and family members
• Accurately document observations, interventions, and evaluations in patient's electronic chart
  
  
  

Physical Therapist (PT) Skills and Qualifications:

• Current NJ PT License
• Reliable transportation and Driver’s License
• Strong interpersonal and customer service skills.
• Ability to learn / navigate electronic documentation systems (HCHB or others)
• Bilingual a plus
• Additional training certificates a plus
