River Edge offers outpatient services at our Emery Highway-Macon, Monroe County and Baldwin County locations. Outpatient services include behavioral health, nursing, and psychiatric assessment, stabilization and medication maintenance services as well as crisis intervention services as well as awareness, education and recovery groups.
River Edge is seeking full-time Case Managers with a bachelor’s degree in social work, psychology, or related fields to serve the Bibb County area. Case Managers connect individuals to community resources and organizations to help meet their basic needs. They also provide comprehensive service coordination and skills training to support individuals with mental illness, substance abuse, or intellectual disabilities in developing and implementing recovery plans, empowering them to live independently and achieve their goals.
FLSA Classification: Hourly, Non-Exempt

- Assisting clients in locating and utilizing community resources such as legal, medical, and financial assistance, and providing referrals as needed.
- Offering resource linkage and skills training to clients in personal care homes and transitional housing, including pre-release and release planning.
- Maintaining communication with other social service agencies and healthcare providers to exchange information and monitor clients' overall progress.
- Delivering skills training for recovery management, daily living activities, and social/interpersonal skills as needed.
- Preparing regular case reviews detailing progress toward treatment goals.
- Performing other duties as assigned.
- Bachelor’s degree in social service field (Social Work, Sociology, Psychology, Human Services) or related field.
- Proficiency in Microsoft Office Suite and the ability to learn relevant software applications, including electronic medical records.
- A good motor vehicle record and the willingness to transport clients using personal vehicles.
- Pass a satisfactory background clearance and pre-employment drug test.
- Provide two professional references (former employers only, no family members).
