Position Summary: A WISe Care Coordinator works with youth and families enrolled in the WISe program and functions as a core member of a collaborative care team. The Care Coordinator facilitates the Child and Family Team (CFT) and is responsible for guiding the team through the phases and activities of WISe. The Care Coordinator partners with youth, family, natural support and other community service providers to promote safe, stable and healthy families. This position provides community-based services, including meetings occurring in the family’s home or community setting of their choice. This position requires the ability to work flexible hours including evenings and 24/7 On-Call availability on a rotating basis.

Responsibilities: The primary duties and responsibilities of the WISe Care Coordinator include, but are not limited to:
- Maintain a caseload of up to 15 youth and their families. 2. Assemble and engage a Child and Family Team (CFT) consisting of both formal and informal support systems.
- Provide CFT members with an overview of the WISe process and clarify their role and responsibilities as a team member in this process.
- Assess the youth and families’ strengths and needs.
- Coordinate and facilitate the meeting of the Child and Family Team (CFT) and develop an individualized Plan of Care based on the family’s strengths, needs and their preferred solutions.
- Act as the central point of communication for the WISe team.
- Identify potential barriers to participation such as transportation needs and work schedules. Collaborate with the youth, family, and other team members to identify solutions to remove barriers.
- Develop an individual support plan with each enrolled youth and their family. Elicit feedback from the youth/family about the effectiveness of the plan and updating the plan regularly to maximize usefulness to the youth and their family.
- Increase the “natural supports” in CFT membership and the youth/family’s integration into their community. This is accomplished by getting to know the family history, culture, and resources, and by helping the youth and family to identify and engage potential supports.
- Prepare for meetings: Develop a meeting agenda with the youth, family, and other CFT members; schedule meetings at a place/time that is accommodating (comfortable and convenient) to the youth and family and available to all CFT members; prepare visual aids or tools to facilitate the meeting process; and inform all CFT members of the date, time and location of each meeting.
- Contact CFT members who are unable to attend a meeting, in advance, to elicit their input.
- Ensure all CFT members receive an updated copy of the Plan of Care, Child Family Team meeting activities, discussions and task assignments within 7 days after the CFT meeting.
- Maintain team focus on scope of work for the WISe team and progress/movement toward transition.
- Participate in the 24-hour On-Call rotation for the WISe Program.
- Transport youth/families as necessary and travel to various community locations for meetings, interventions and other identified needs.
- Other duties as assigned by the Program Supervisor or designee
- Complete all progress notes and other documentation required for the youths’ medical record within the timelines outlined by LCSNW.
- Attend and participate in routinely scheduled department and agency staff meetings.
- Complete all the required training.
- Model and encourage direct, professional communication and promote a positive work environment.
- Meet or exceed productivity expectations for the role.
- Recognize the value and strengths of all cultures and maintain a professional problem-solving attitude while dealing with cross-cultural challenges or conflicts.
- Function in accordance with the LCSNW personnel and administrative policies and procedures and administrative practices of the Columbia Basin office.
- Demonstrate and implement ethical decision making in accordance with the Columbia Basin office Ethics Training, provided upon hire
- Knowledge, education, and training in the areas of human behavior and mental health; awareness of individual differences in ability, personality, and interest.
- Dependable, flexible, and punctual performance, including managing one's own schedule within a 37.5 hour full time work week.
- Ability to work in an unstructured and unpredictable setting, ability to adapt to change
- A strong work ethic and a focus on serving youth and families.
- Able to work respectfully and cooperatively with a variety of persons, including coworkers, supervisors, community partners, the public, volunteers, and enrolled youth and their families.
- Able to stay calm and present in tense situations.
- Able to actively support and uphold the mission, beliefs, and values of Lutheran Community Services at all times.
- Proficient use of computer-based programs; preferred experience with Microsoft Office, Gmail and Google Calendar or similar web-based and computer programs.
- BA/BS in Social Work, Psychology, or a related field is preferred.
- OR Certified Peer Counselor credential, including documentation of completion, with a minimum one year of experience.
- 1-2 years experience working in a related field or program is preferred.
- Obtain Washington State Agency Affiliated Counselor Registration or related professional license (Within 60 Days)
- CPR/First Aid (Within 90 Days)
- Washington State CANS certification. (Within 90 Days)
- Clearance through the Washington State Patrol (background check) and verification of ability to work around vulnerable populations is necessary.
- Clearance of Motor Vehicle Report to meet LCSNW insurance requirements.
- Valid driver’s license, proof of Insurance and reliable transportation is required
