Job Summary:
Provides nursing care to patients under the direction of a Registered Nurse or physician/advanced practice provider. Utilizes observational skills and assists medical staff to ensure optimal care within established policies, procedures, and standards. Demonstrates performance consistent with the Mission, Vision, and Strategic Plan of the organization, showing quality and effectiveness in work habits and clinical practice.
Essential Responsibilities:
- Exhibits excellent customer relations with patients, visitors, providers, and co-workers; shows courtesy, compassion, and respect; conforms to the critical service standards established for the department.
- Provides therapeutic patient care as established by a physician/advanced practice provider or registered nurse within the scope of practice.
- Transcribes physician/advanced practice provider orders as directed by policy.
- Communicates directly with a registered nurse to institute changes in the plan of care; assists in developing a nursing care plan.
- Administers specified medication appropriately and accurately following providers' orders and/or standing orders within scope of practice.
- Executes treatments such as urinary catheterizations, IV placements, bladder irrigations, and suctioning procedures, adhering to infection prevention and control policies.
- Adheres to high levels of disinfectant and point of use standards.
- Performs related patient care functions including taking vital signs, wound care, and instructing patients in self-administered treatments.
- Assists medical staff with examinations and treatments by providing necessary instruments and support.
- Observes and documents patients' conditions, initiates emergency procedures as necessary, and reports findings to the registered nurse/advanced practice provider.
- May start IV flow rates and perform venipunctures with Certifications.
- Collects and processes specimens per policy and procedures.
- Provides non-complex education following the registered nurse/advanced practice provider/physician's plan.
- Ensures cleanliness of nursing modules and proper functioning of patient care equipment, providing a safe environment.
- Utilizes effective communication skills.
- Works in other departments as required to maintain established standards of care and safety.
- Performs departmental back-office workflows including patient outreach and nurse visits.
- Participates in staff meetings and in-service programs.
- Assists in ordering and stocking supplies and equipment.
- Accepts verbal and telephone orders from physicians/advanced practice providers.
- Supports patient communication and message management.
- Administers therapies and documents care in accordance with policies and procedures.
- Performs other related duties as assigned, including orientation of new employees.
Basic Qualifications:
Experience
- Minimum Twelve (12) months LVN direct patient care experience within the last three (3) years.
Education
- High School Diploma or GED
License, Certification, Registration
- Vocational Nurse License (California)
- Basic Life Support
Additional Requirements:
- Extraordinary customer relations and quality service are critical.
- IV Certification and Blood Withdrawal Certification preferred.
- Bilingual (English/Spanish) QBS Level II required.
Preferred Qualifications:
- Peds experience preferred.
Notes:
- Sunday 8:30am - 5:00pm; Monday-Thursday 11:30am - 8:00pm.
- Must have passed the bilingual test (within the last 12 months) or be active in the QBS program.
Job Location: California, Fontana, Fontana Medical Offices 1 & 2.
