Who we are: La Clínica de La Raza is a community-based health center committed to providing culturally appropriate, high-quality, and accessible health care to the diverse communities of the San Francisco Bay Area. We have spent over 40 years advocating for and creating a health home for the many that have been denied access to care. As healthcare activists, we are dedicated to making sure individuals without health coverage receive the same level of quality healthcare as those with it. Our goal is to address the economic and social factors that affect health, often requiring us to provide services outside our centers to connect individuals to preventive and primary care. Be part of an excellent care team: At La Clínica, you will provide primary care in an outpatient setting, allowing for comprehensive medical care practice. You will serve a diverse and medically complex patient population, aiming for optimal health outcomes. Major Areas of Responsibility include but are not limited to:
Healthcare Delivery
- Provide medical triage, care management, and patient education to a panel of patients.
- Make referrals to specialty providers and follow-up to address patients’ bio-psychosocial needs.
- Deliver evidence-based care to improve health outcomes, patient experience, and resource efficiency.
- Perform routine, acute, urgent, preventive, curative, and rehabilitative care and determine appropriate regimens.
- Engage in routine and preventive primary care, chronic illness care, urgent care, and telephone triage.
- Prescribe medications and order medical diagnostic tests.
Patient Experience
- Deliver well and chronic disease care with cultural humility to diverse patients of all ages.
- Adapt to patients’ needs, including evening and Saturday clinics.
- Develop positive rapport with patients and families to enhance the provider/patient relationship.
- Conduct in-home patient assessments as needed.
Collaboration & Mentorship
- Participate in cross-functional teams for assessment, management, and treatment of complex cases.
- Consult with physicians and suggest improvements for clinic flow and service delivery.
- Supervise medical assistants by providing patient consultations.
Minimum Job Requirements
Knowledge
- Experience with Electronic Health Record programs, such as NextGen or Epic.
- Interpersonal competence and thorough knowledge of evidence-based care.
- Cultural competence in diverse/low-income communities.
- Familiarity with Electronic Panel Management.
Abilities
- Effectively handle complex patient profiles and maintain good staff relationships.
- Physical stamina for heavy lifting (50lbs).
- Work independently with good judgment; communicate effectively.
- Work under pressure and as part of an interdisciplinary team.
- Support diversity, equity, and inclusion.
- Maintain accurate patient records.
- Demonstrate time management skills and evidence-based medical approaches.
- English/Spanish proficiency preferred but not required.
Experience and Other Certifications
- Current BLS Certificate, board certification required.
- Valid California license and DEA.
- Nurse Practitioner: License from California Board of Registered Nursing, national certification, and completion of a Nurse Practitioner Program with 2 years clinical experience.
- Physician Assistant: License from California Physician Assistant Committee, approved program graduation, certification examination, and 2 years full-time clinical experience in family medicine or geriatrics.
