Pay Rate: Up to $60,000 per year

Key Responsibilities:

• Conduct insightful interviews with residents and families to collect essential social history.
• Empower residents and families to actively participate in developing social service programs.
• Collaborate to devise innovative solutions for social and practical challenges, including financial assistance and discharge planning.
• Work seamlessly with various departments to design impactful social service programs.
• Actively participate in facility surveys conducted by authorized government agencies.
  
  
  

Minimum Qualifications:

• At least 2 years of experience in a Skilled Nursing Facility (SNF).
• Proven ability to collaborate across departments and engage with diverse populations.
