Hospice of San Joaquin is the oldest not-for-profit hospice of 40 years, serving all of San Joaquin and Stanislaus Counties. We are committed to ensuring the quality of the journey from serious illness through end of life, focusing on patients and their caregivers. We seek a dynamic, self-motivated Bereavement Services Manager for a full-time position.
Position Purpose/Summary:
Identifies the bereavement needs of hospice families and community clients, coordinating, supervising, and provisioning bereavement services.
Coordination/Development of Bereavement Services
Ensures the bereavement needs of hospice families and community clients are met in accordance with Hospice of San Joaquin's policies, Federal and State regulations, and current hospice standards.
- Develop and maintain hospice policies and procedures for bereavement services.
- Facilitate case presentations and assignments among bereavement team members.
- Plan, schedule, supervise, and/or facilitate bereavement support groups.
- Conduct and supervise bereavement quality improvement activities.
- Provide consultation to healthcare staff and collaborate with community agencies regarding bereavement and loss issues.
- Supervise bereavement services provided by staff and volunteers, maintaining consistency with hospice philosophy.
Supportive Counseling Services
Provides supportive counseling and/or referrals to meet clients' bereavement needs.
- Assess hospice families and develop a bereavement plan of care 4-6 weeks after the patient's death.
- Assess community clients at the time of referral and develop a support plan.
- Provide goal-directed counseling and/or referrals to bereaved clients.
- Identify and coordinate referral resources to support bereavement plans.
- Make independent decisions regarding psychosocial and bereavement issues.
Expansion of Community Services
- Identify and assess needs in grief and loss support.
- Develop new programs to meet the needs of hospice clients and the community.
Education
Enhance continuity of bereavement services and community awareness.
- Coordinate training and in-service for bereavement volunteers.
- Provide feedback to the Chief Clinical Officer on timely topics for education and discussion.
- Advocate for hospice and bereavement services through speaking engagements.
Interdisciplinary Team Collaboration and Communication
Stay apprised of family dynamics before a patient’s death and assist IDT members with issues of grief and loss. Participate in monthly bereavement team meetings.
Documentation
Participate in professional educational activities for personal growth and job satisfaction.

Job Qualifications
- BA/BS degree in Social Work, Sociology, Counseling, or related field; Master's Degree preferred.
- Understanding of individual, child, and family development, and the influence of culture and values in grief counseling.
- Knowledge of group processes and peer counseling with an IDT approach.
- Skills in decision-making, program planning, and administration.
- Experience working with individuals in traumatic life experiences.
- Ability to supervise, train, and evaluate employees and volunteers.
- Organizational and prioritization skills.
- Knowledge of child and elder abuse reporting requirements and laws regulating hospice practice.
Other Qualifications
- Valid California driver's license and automobile insurance meeting State standards.
- Able to sit for long periods and maneuver throughout the community.
- Visual/hearing ability to comprehend communication.
- Manual dexterity for computers and filing.
- Ability to carry and move materials for workshops and support groups.
