Home Health RN

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team in providing exceptional home health care services in Medford. As a Home Health RN, you will have the opportunity to make a meaningful impact on the lives of patients by delivering personalized care in the comfort of their homes. Your role will involve collaborating with a multidisciplinary team to ensure the highest standards of care.

Key Responsibilities

• Conduct comprehensive assessments of patients’ health status and develop individualized care plans.
• Administer medications, treatments, and procedures as prescribed by physicians.
• Monitor and document patients' progress, reporting any changes to the healthcare team.
• Educate patients and their families on disease management, treatment options, and preventive health measures.
• Coordinate with other healthcare professionals to ensure seamless care and support.
• Maintain accurate and timely patient records in accordance with regulatory and organizational standards.
  
  

Qualifications

• Current RN license in the state of practice.
• Minimum of one year of nursing experience, preferably in home health or a related field.
• Strong clinical skills with the ability to work independently in a home setting.
• Excellent communication and interpersonal skills.
• Ability to manage time effectively and prioritize tasks.
