Summary

Perform diversified job duties to support assigned individuals participating in the program to achieve successful service outcomes. Act as a mentor, trusted ally, and role model. Work as part of the interdisciplinary team to assist adult individuals with disabilities and/or other challenges in identifying and making choices about their social, vocational and personal goals; and in choosing, getting, and keeping individual competitive employment.

Essential Duties and Responsibilities

  • Depending on the Program's design, may assist fellow supported employment team members or may directly fulfill duties and responsibilities.

May carry a caseload of up to 20 individuals receiving employment services at any time.

Coordinate intake process for new referrals and/or interdepartmental transfers, including review of referral materials/Master File, conducting a comprehensive intake interview, development of an Individual Recovery Plan (IRP), and completion of required documentation.

Implement the IRP, ensure employment readiness, and facilitate the smooth and timely transition to employment.

Develop and maintain relationships with community organizations, employers, and other contacts to ensure resources are available to individuals.

Schedule, facilitate, and/or participate in interdisciplinary team meetings to enhance and review the IRP.

Provide information and insight on strategies for successful employment outcomes.

Prepare for and participate in scheduled clinical team meetings.

Empower individuals to maximize their input regarding services/supports through self-advocacy activities.

Assist in training programs such as job seeking, job retention, and relevant life skills.

Coordinate and implement peer support groups and activities.

Review information about individuals to understand their needs related to vocational planning and job development.

Assist in developing professional resumes for job searches.

Ensure job placements that match individual's skills, needs, and vocational goals.

Train individuals to perform work tasks appropriately and productively.

Work with employers to ensure a safe and accessible work environment.

Provide crisis intervention and use approved behavioral management techniques.

Complete required documentation for agency, state, and federal standards.

Use a computer to complete service documentation in program data management systems.

Ensure necessary authorization for services and complete related billing.

Assist individuals with transportation as required.

Facilitate discharge planning and follow-up regarding transfers of individuals.

Perform other related essential duties as required or directed.

Qualifications

Bachelor’s degree in a related field or equivalent experience; knowledge and experience in behavioral health services required.

Language Skills

Ability to read and interpret documents; write professional reports; communicate individual status, problems, or needs.

Mathematical Skills

Ability to perform basic mathematical operations and compute rates, ratios, and percentages.

Reasoning Ability

Ability to solve practical problems and assist individuals with social behavior to facilitate integration and inclusion.

Certificates, Licenses, Registrations

Must obtain Recovery Support Specialist certification; valid Connecticut driver's license required.

Other Skills and Abilities

Ability to obtain certification in physical/psychological management training, CPR, and First Aid.

Physical Demands

Regularly required to stand, walk, and sit; ability to lift and/or move up to 50 pounds.

Work Environment

May work near moving mechanical parts; exposed to outside weather conditions.
