Summary

The At Home Service Coordinator coordinates retirement community services for its members and their homes. These services may include but are not limited to, preventative and routine home maintenance, housekeeping, health club privileges, wellness programs, 24/7 emergency response, social opportunities, lunch outings, meal programs, group discounts and transportation. The At Home Service Coordinator will also coordinate or perform personal care.

Essential Functions:

Service Coordination

Coordinating outside services when needed. All essential functions will be performed with empathy, dignity and respect of the member and will respect and accommodate their individual choices whenever possible. All essential functions will be done in support the Mission and Ministry of St. John’s.

Service Providing

Ensuring the basic member benefits are installed and/or available. These include but are not limited to; emergency response service, health and wellness access, monthly home maintenance, and providing regular social opportunities.

Personal Care

At Home Service Coordinator will arrange for or provide personal care for members who elect this service:

  • Ensures that the member has the appropriate products available for personal care.
  • Assigns or is responsible for any personal care needs for members as needed.

Housekeeping

At Home Service Coordinator will arrange for or provide housekeeping for members who elect this service:

  • Responsible that the member’s home is cleaned per the expectation within member’s individual service contract.
  • Ensures that the member has the appropriate products available to properly clean.
  • Will work directly with members and housekeeping staff (if appropriate) to resolve issues and maintain quality assurance.

Landscaping

At Home Service Coordinator will arrange for or provide for members who elect lawn and snow removal services:

  • Provide maintenance and annual tune up when using available homeowner equipment
  • Ensures that the member has the appropriate supplies and equipment as needed.
  • Assigns or is responsible for all members’ lawn, garden, and snow removal needs.

Home Maintenance

At Home Service Coordinator will arrange for or provide monthly preventive maintenance and repairs requested by members.

  • Ensures that maintenance is performed in a timely and cost-effective manner.
  • Ensures that the member has the appropriate supplies and equipment as needed.
  • Assigns or is responsible for decorating the member’s residences for all holidays, and events within member’s individual service contract.

Meal Preparation

At Home Service Coordinator will arrange for or provide for members who elect in home meal preparation:

  • Meal planning and preparation
  • Coordinates grocery orders, shopping, and takeout orders
  • Ensures that the member has the appropriate supplies and equipment as needed.

Transportation

At Home Service Coordinator will arrange for or provide for members who elect transportation options.

Personal Emergency Response System

At Home Service Coordinator will be responsible for initial setup, assessments, upkeep, and other duties of Personal Emergency Response Systems.

Activities

At Home Service Coordinator will arrange for or provide for members who elect activities and health and wellness programs.

  • Implements a monthly schedule of diverse member-centered activities.
  • Creates and communicates an online monthly activity calendar and upcoming events.

After-hours availability

At Home Service Coordinator will arrange for or provide after hour availability in partnership with other At Home Service Aides and managers to ensure 24/7 availability to members and will rotate call.

Communication

  • Meet regularly with community-based organizations (CBOs), local leaders and other notables to keep them informed about the At Home services.
  • Develop flyers, posters and other materials as needed.

Knowledge, Skills/Abilities, Work Experience:

Education: Must have at minimum a high school diploma or GED.

Work Experience: At least one year of direct care experience with older persons or 6 months CNA experience in a Skilled Nursing setting preferred. Must complete the SJU CNA Course within 6 months from hire date. Must obtain certification within 120 days of class completion.

Certifications: Recent experience as a certified nursing assistant preferred.

CPR certified preferred or must obtain certification with SJU within 6 months.

Montana Driver’s License with clean record.

  • Communication through use of technology helpful.

Complexity/Difficulty:

  • This position requires the ability to function independently in performing assigned duties. The incumbent needs to be able to respond to management, staff and members in a timely and professional manner. Incumbent needs to perform job duties by following established procedures and report any problems or concerns to direct supervisor.

Physical Demands:

  • Requires heavy physical work more than 30% of the time, including lifting up to 25 pounds regularly and up to 50 pounds occasionally. Ability for standing, climbing a ladder/comfortable with heights, pushing, pulling, lifting, walking, and bending for prolonged periods. Must be comfortable driving a company-owned vehicle.

  • Home-like setting
  • Work involves some exposure to unusual elements such as extreme temperatures, fumes, unpleasant odors, and/or loud noises.
  • Work involves exposure to hazards, blood borne pathogens, or physical risks, which require following basic safety precautions.
  • Work could involve exposure to plants and animals within the environment.
