Job Title: Clinical Assessor
Pay: (per approved assessment)
The Clinical Assessor is responsible for conducting comprehensive evaluations of Individuals' medical, psychological, or emotional status to determine the appropriate care, treatment, and services required. They work closely with individuals, families, and multidisciplinary teams to develop and implement individualized care plans.
Key Responsibilities:
Individual Evaluation:
Clinical Assessors may work in hospitals, Individual clinics, residential treatment centers, rehabilitation facilities, mental health centers, or community health agencies. The role may involve working with a variety of Individual populations, including those with mental health disorders, disabilities, or substance abuse issues.

The Clinical Assessor is responsible for conducting comprehensive evaluations of Individuals' medical, psychological, or emotional status to determine the appropriate care, treatment, and services required. They work closely with individuals, families, and multidisciplinary teams to develop and implement individualized care plans.
Key Responsibilities:
Individual Evaluation:
- Conduct initial and ongoing assessments of Individuals to determine the need for medical or mental health care services.
- Review Individual history, symptoms, and relevant diagnostic tests to assess their condition.
- Use clinical tools and standardized assessments to evaluate the severity and scope of Individuals’ issues (e.g., mental health, physical health, or functional ability).
- Care Plan Development:
- Collaborate with healthcare providers to develop appropriate care plans based on assessment findings.
- Recommend specific treatments, interventions, or programs tailored to Individuals’ needs.
- Ensure that care plans are updated regularly based on ongoing evaluations and progress.
- Documentation:
- Maintain accurate and thorough records of assessments, care plans, and Individual progress.
- Document all clinical findings, interventions, and communications in a timely manner according to regulatory standards.
- Prepare reports summarizing assessment results for other healthcare providers or case managers.
- Collaboration and Consultation:
- Work with multidisciplinary teams to provide appropriate care.
- Participate in care conferences and team meetings to discuss Individual progress and modify care plans, as needed.
- Provide recommendations to families and caregivers regarding support and care options.
- Individual Education:
- Educate individuals and their families about their condition, treatment options, and available services.
- Advise on resources, including community support systems, to promote Individual well-being.
- Compliance and Standards:
- Ensure that all assessments and interventions are in compliance with regulatory guidelines and organizational policies.
- Stay up-to-date with current clinical best practices and research in the relevant area of healthcare.
- Crisis Management:
- Assess and respond to crisis situations, particularly in the mental health or behavioral health settings, and provide immediate intervention or referrals when necessary.
- Education: At a minimum Masters Degree in Human Services related field.
- Experience: Several years of clinical experience in the mental health field
- Licensure/Certification: LMHP-E or Fully Licensed
- Skills:
- Strong clinical and diagnostic skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with diverse healthcare teams.
- Proficiency in using clinical documentation systems and assessment tools.
- Empathy, attention to detail, and problem-solving abilities.
