Key Responsibilities:?

• Routinely travel to patients' homes to deliver care, educate, conduct evaluations, and offer ongoing support, ensuring personalized and effective treatment.
• Demonstrate comfort and professionalism when working in diverse patient home environments, ensuring a compassionate and safe care experience for each individual.
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;?
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;?
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory; management, admission documentation, creating patient charts, etc.);?
• Maintains a positive and respectful attitude to all patients and colleagues; and
• Medication administration and documentation?
  
  

Requirements:?

• Valid RN license
• Valid state driver's license
• 2 years of RN experience required
• 1 year of hemodialysis required
• 1 year of Nxstage machine required
• BLS certification; and
• Ability to lift up to 35 pounds
• This is a safety sensitive position
