Licensed Behavioral Health Clinician / Care Advocate
The overall purpose of the role is to improve the enrollee’s ability to remain stable in the community and out of the hospital. Care Advocates engage people in the treatment process and assist them to access the appropriate community services so they can pursue their treatment goals in the community and avoid institutionalization. This position serves as a direct clinical liaison between health plan staff and the clinical network. The program fosters a collegial and mutually beneficial relationship between the providers and payers of medical and behavioral health services.
Location: Bronx & Lower West Chester, NY - 80% field-based, 20% remote.
Schedule: Monday – Friday, 8 am to 5 pm.
Patient Population: Adult Behavioral Health
Primary Responsibilities:
- Identify and provide community service linkage
- Assist enrollees with aftercare appointments
- Provide follow-up as determined by individual member needs
- Work with enrollees to identify gaps in care and problem solve for successful connection to services
- Review IP Census daily for recent re-admits
- Attend facility meetings to develop aftercare plans
- Manage high risk enrollees' follow-up
- Meet with community providers as company liaison

Required Qualifications:
- Active, unrestricted, independent license in New York:
- Licensed Mental Health Counselor (LMHC)
- Licensed Marriage & Family Therapist (LMFT)
- Licensed Clinical Social Worker (LCSW)
- Licensed Psychologist (LP)
- Registered Nurse (RN) with 10+ years of Behavioral Health experience
- 2+ years of related experience
- Proficient computer skills and Excel knowledge
- Access to secure, high-speed internet at home
- Reliable transportation for daily travel
Preferred Qualifications:
- Experience in a medical/behavioral setting
- Dual diagnosis experience
- Case Management experience
- Field-based experience
- Experience with low-income populations
- Knowledge of city and state agencies and practices
