Position Summary: The Intake Coordinator plays a vital role in facilitating the admission of patients into home health services. This position is responsible for processing referrals, verifying insurance eligibility, coordinating with healthcare providers, and ensuring a smooth transition for patients into home care. The Intake Coordinator must demonstrate excellent communication skills, attention to detail, and a strong understanding of home health regulations and procedures.
Schedule: Thursday-Monday, 9am-5pm

Referral Management:
- Receive, review, and process referrals from hospitals, physicians, and other healthcare providers.
- Ensure all required documentation is received, complete and meets regulatory and organizational standards.
- Communicate with referral sources to gather any missing or additional information needed for patient intake.
- Communicate and collaborate with the community liaison team ensuring referrals are properly addressed in a timely manner, may need to work with multiple members at a time.
- Meets or exceeds daily productivity requirements established by the department. Key Performance Indicators (KPIs)
- Verify patient eligibility, insurance benefits, and authorization requirements.
- Collaborate with clinical teams to determine patient needs and appropriate care services as needed.
- Schedule initial visits and coordinate with Case Managers for timely admissions.
- Serve as the primary point of contact for patients, families, and healthcare providers regarding the intake process.
- Provide clear and compassionate communication to patients and families about services and expectations.
- Address any concerns or barriers to care and escalate issues as needed.
- Overcome objections to home health services by the patient, family or responsible party.
- A variety of communication channels are used to facilitate the flow of information in a timely manner, including but not limited to face-to-face conversations, phone calls, emails and faxes among internal and external partners.
- Handle complex and/or sensitive matters with careful judgment and tact consistently, escalating to leadership as appropriate.
- Ensure all intake documentation is accurately recorded in the electronic health record (EHR) system, CRM, and all logs.
- Maintain compliance with Medicare, Medicaid, and private insurance guidelines for home health admissions.
- Keep up-to-date with industry regulations and organizational policies.
- Ensures patient confidentiality is met by adhering to Health Insurance Portability and Accountability Act (HIPAA) and Protected Health Information (PHI) rules and regulations

Position Qualifications:
- Two years of direct experience in Referral Intake, Patient Registration/Admissions or a related field is required, preferably in a Home Health environment.
- Strong organizational skills, attention to detail, proficiency in EHR systems, and understanding of HIPAA regulations.
- Excellent written and verbal communication skills.
- Proficiency in Google Suite and other relevant software.
- Ability to multi-task, balance workload, and prioritize to meet deadlines
- Must be able to complete tasks in a noisy environment
- Must be able to work in a fast-paced environment
- Must be able to adjust to constantly evolving deadlines
- Must adhere to strict deadlines
- Must be able to adhere to and apply established protocols in a timely manner
- Repeating motions that may include the wrists, hands, and/or fingers.
- Remaining in a stationary position, often standing or sitting for prolonged periods.
