Job Summary:
The Home Navigator will be responsible for meeting with parents and families in the home environment and providing parent education, child development information, and family support. Through personal home visits, the home navigator will promote the health and development of children from pregnancy to kindergarten. Home Navigators utilize the nationally recognized “Parents as Teachers” (PAT) curriculum to deliver quality home-based early childhood education and provide developmentally age-appropriate resources for effective parent-child interaction. Home Navigators will utilize a trauma-informed approach to provide support to families and can recognize and respond to the varying impact of traumatic stress on children and their families. Home Navigators are required to establish and develop collaborative partnerships and connect families to resources.
Essential Functions
Knowledge, Skill, and Experience:
Education: A bachelor’s degree in social work, Child Development, Psychology, or another related field is preferred; candidates with a high school diploma or equivalent and 2+ years’ work experience in home visitation may be considered in lieu of a degree.
Skills: Must have strong communication skills, written and verbal, interpersonal skills, planning/organizing, information gathering, identifying solutions, summarization, listening, goal setting, multi-tasking, teamwork, collaboration, initiative, and ability to make independent decisions within guidelines, and follow the code of ethics
Experience
Typical Working Conditions: Staff are required to visit families in their homes. EPU does not have control over the condition of all the environments to which staff may be exposed.
Equipment Used: Personal vehicle for local travel, computers, printers, telephones, and fax machines.
Essential Physical Tasks: Working with children and their families may require the ability to sit on floors, stand, stoop, crawl, lift, carry, push, and pull.

Essential Functions
- Required to complete the Parents As Teachers Foundational and Model Implementation Trainings before delivering PAT services.
- Provide in-home case management and utilize community resources to link families to appropriate services, conduct in-home teaching and supportive problem-solving.
- Utilize the Parents As Teachers curriculum and engage parents in parent-child interaction activities, and increase knowledge of children’s development
- Work with adults and their young children to develop, monitor and review goals with each family
- Make regularly scheduled home visits on all families assigned in accordance with the level of intensity based on the need as determined by the assessments and home navigator observations
- Must be able to plan, provide, and document personal visits focused on parent-child interaction, development-centered parenting, and family well-being.
- Complete an initial and annual family-centered assessment with each family
- Complete at least initial and annual developmental screening and health review for each enrolled child
- Must be able to establish a relationship with families and empower them by building on their strengths
- Maintain and manage a caseload of approximately (15-20) families.
- Help parents and children transition to other services as needed
- Maintain and submit all required family documentation and data in a timely manner
- Must work independently and be self-motivated.
- Participate in at least two hours of reflective supervision monthly and at least two hours of staff meeting monthly
- Obtain competency-based professional training and renew home navigator certification annually
- Participate in continuous quality improvement
- Facilitate group connections to give families an opportunity to build social connections with one another.
- Required to understand all EPU policies and procedures and maintain proper confidentiality.
- Maintain, manage, and process client records and information in a confidential manner that meets HIPAA and other applicable confidentiality regulations.
- Position requires data entry utilizing the Apricot and Visit Tracker database, answering phones, and various computer activities.
- Position requires local travel daily
- Keep an accurate mileage log when utilizing your vehicle for home visits, meetings, and/or training.
- EPU facility hours are Monday – Friday; 8:30 AM – 5:00 PM; with some exceptions based on family’s needs. May be required to work evening.
- Must perform other duties as they are assigned by the Director, Home Visitation Manager, Site Supervisor, or President/Chief Executive Officer.

Experience
- 2+ years’ experience working with high-risk families and families with young children
- Case management and a strong understanding of community resources required
- Ability to work independently and be self-motivated
- Must have experience working with a diverse population and be a team player
- Experience in working with families to empower them to engage in problem-solving.
- Must hold a valid California Driver's license and good driving record with no serious violations for the previous two-year period and be able to provide proof of current automobile liability insurance.
- You must have reliable transportation to visit families in their homes.
- Bi-lingual in Spanish is a plus.
- Computer skills, including online research, email, internet, and word processing and knowledge of Microsoft Office products.
