Primary Objective

The City of Bloomington Public Health Division is seeking an individual to join our efforts in identifying, treating, and preventing substance use and addressing mental health in the Bloomington, Edina, and Richfield (BER) communities we serve. We are seeking a motivated and experienced professional to fill a crucial role in the development, implementation, and oversight of community-based education programs aimed at addressing substance use and mental health issues. The ideal candidate will have a strong background in community-based health education, community engagement, and a commitment to enhancing the well-being of individuals and families.

Examples of Duties

Responsibilities

  • Provide and manage wellbeing programs that improve community mental health.
  • Use data to identify community needs and assets prior to planning, implementing, monitoring, and evaluating public health programs and activities.
  • May administer fiscal resources for health education programs.
  • Community Events: attend, support, plan, coordinate, and facilitate.
  • Prepare grant applications and reports.
  • Serve as the point person and provide coordination for mental health work in the Division.
  • Serve as a liaison for the health department in the community by participating on community coalitions, committees, and task forces.
  • Coordinates development and implementation of community-based substance use and mental health initiatives that align with organizational goals and community needs in collaboration with Substance Use Prevention, Wellbeing & Policy Supervisor and Population Health and Planning teams.
  • Leads and coordinate immediate, short-and long-term planning and implementation process with Bloomington Public Health Division and City leadership, residents, and community partners to identify Well-being strategies, priorities, recommendations, and outcomes for work plans geared at prevention of substance misuse and its impacts in the BER community.
  • Establish and manage internal and external partnerships and build relationships with communities, community leaders, and those who have direct experience with substance misuse prevention and harm reduction efforts to ensure that all voices are represented in planning and implementation.
  • Coordinates and facilitates meetings with diverse stakeholders, including scheduling, agenda development, logistical planning, meeting minutes, and reporting.
  • Coordinates the development and implementation of individual and programmatic work plans that support timely and impactful program outcomes.
  • Collaborates with SUMH Supervisor to develop and manage program work plans, budgets, and reporting, ensuring fiscal monitoring and compliance with funding requirements.
  • Collaborates with internal and external stakeholders, community partners, and local organizations to maintain a supportive network for sustainable substance use and mental health initiatives.
  • Serves as a key point of contact for the ongoing Well-being, mental health, cannabis, and commercial tobacco prevention work both locally and across the State.
  • Seeks out resources to support the City’s Mental Health and Wellbeing and other substance misuse prevention efforts in partnership with the Public Health Division’s process to manage funds.
  • Demonstrates a consistent ability to apply a health equity lens to all aspects of substance use and mental health work and other work across the Division that impacts the community and intersects with the purpose of this role.
  • Collaborates with the Substance Use/Mental Health team and other partners across the Division and City to facilitate community outreach and education initiatives to raise awareness and reduce stigma surrounding substance use and mental health.
  • Analyzes and monitors community risk factors, providing regular reports based on program effectiveness and feedback to develop recommendations that enhance program outcomes and impacts.

Essential Duties

  • Assessment & Surveillance
    • Plans assessments, defining the purpose and scope of the assessment.
    • Identifies priority populations, existing and available resources, policies, programs, practices, and interventions; designs process, impact, and outcome evaluations of the intervention.
    • Obtains and identifies primary data, secondary data, and other evidence-informed sources and determines its validity and reliability.
    • Manages collection and evaluation of data using appropriate technology; conducts community and participant surveys to ascertain health needs, develop desirable health goals, and determine availability of services.
    • Analyzes the data to determine the health of the priority population(s) and the factors that influence health.
    • Synthesizes assessment findings to inform the planning process.
    • Interprets data. Uses findings to improve programs.

  • Community Partnership Development
  • Engages priority populations, partners, and community organizations for participation in the planning process.
  • Serves a role in local coalitions, regional initiatives or groups, and state level coalition groups; participates in coalition meetings.
  • Coordinates relationships with partners and community organizations. Provides training to expand the capacity of partner organizations.
  • Promotes and represents programs at community events and at internal activities.
  • Recruit health care providers/clinicians, community and faith-based organizations, and other partners to promote programs to clients.
  • Serves as a facilitator in coordinating meetings and distribution of information.
  • Works with community members to assist with cultural and linguistic educational materials for target population.
  • Assists in identifying local health resources that could be mobilized to improve substance use prevention and cessation activities.

  • Leads implementation and evaluation of community-based prevention and cessation programs that focus on substance use.
  • Coordinates the delivery of intervention(s) consistent with the implementation plan.

  • Assists with writing, editing, and collecting documentation to ensure full compliance with corresponding grants, including deliverable outcomes.

  • Prepares submission of State and/or Federal grants as appropriate and is responsible for managing program budget, reporting and grant requirements.
  • Researches and prepares grant applications, prepares grant reporting documents.
  • Creates required reports reflected of program services.
  • Maintains inventory of educational materials.
  • Communications
  • Promotes public health programs and services.
  • Provides targeted individual and group level education and outreach for substance use risk reduction, harm reduction, overdose prevention, and naloxone use.
  • Determines health education and promotion interventions, including creating logic models and pilot testing; develops plans and materials for implementation and evaluations.
  • Works with partners and community members to evaluate educational materials to determine appropriateness (cultural & linguistic) for clients being served.
  • Prepares written and digital materials on substance use issues in conjunction with Communications Specialist.
  • Assists in developing marketing plans with Communications Specialist to promote substance use cessation and prevention efforts.
  • Develops effective messaging for target audiences; selects methods and technologies to deliver messages and delivers messages effectively using identified strategies.
  • Prepares and provides presentations in the community.
  • Develops and circulates programmatic newsletter to clients and public.
  • Provides health education to clients in group settings.

Minimum/Desirable Qualifications

MINIMUM QUALIFICATIONS

  • Baccalaureate degree in public or community health, behavioral health, health education/promotion, health sciences, public administration, healthcare, substance abuse, mental health, or related field.
  • 2 years of experience directly related to community-based health education.
  • 2 years of experience in participatory decision-making and/or consensus-building techniques.
  • 2 years of experience developing trusting partnerships across sectors and within organizations.
  • Proficiency in Microsoft Office.
  • Valid driver's license, car insurance, and dependable car OR access to reliable transportation required to perform job duties.

Desirable Qualifications

  • Advanced degree or certification in public or community health, behavioral health, health education/promotion, health sciences, public administration, healthcare, substance abuse, mental health, or related field.
  • Experience working for an organization involved in the response, prevention, and treatment of mental health/substance use.
  • Experience advocating for underrepresented groups of individuals within organizational settings.
  • Experience in public speaking and facilitating.

Anticipated Hiring Range: $ 81 , 890 – $8 6 , 861 Annually

2024 Full Salary Range: $81,890 – $109,171 Annually
