Job Description

HealthPartners is currently hiring for a Senior Value Based Care Clinical Program Manager. The program manager will lead and oversee the development, implementation, and evaluation of programs designed to enhance patient care and outcomes in a value-based care environment. This role will require collaboration with interdisciplinary teams to leverage data insights and drive innovative solutions and interventions in acute, post-acute and ambulatory care settings across the HealthPartners health system.

Required Qualifications

  • Bachelor’s degree in healthcare related discipline.
  • Seven years of population health or value-based care clinical related leadership; with strong program/care model evaluation experience.
  • Understanding of value-based care models, quality measurement, and reimbursement structures
  • Understanding of foundational value-based care strategies and tactics
  • Strong analytical skills with experience in healthcare data analysis and insight application
  • Excellent communication and people skills with the ability to present effectively to all audiences, including front-line team members to executive stakeholders and leaders; as well as group meetings, consensus building and conflict resolution.
  • Ability to work collaboratively in a fast-paced environment.
  • Demonstrated capability to manage multiple projects and activities; successfully navigating implementation and communication timelines.
  • Skilled in making decisions under challenging and ambivalent situations.
  • Demonstrated ability to lead multi-site leaders and multi-site projects effectively and simultaneously.
  • Demonstrated knowledge of and experience with budgetary responsibilities
  • Ability to identify, understand and manage project and business risk.
  • Proficiency in relevant software and data analytics tools

Preferred Qualifications

  • Master’s degree in social work, nursing, or other related healthcare discipline.
  • 10 years of hospital or healthcare related leadership; with progressive and increased relevant leadership in value-based care initiatives.
  • Valid and current discipline license in state of Minnesota or Wisconsin as required for individual’s discipline.
  • Project management, business planning and return on investment evaluation experience; in hospital or healthcare setting preferred.
  • Experience successfully implementing value-based care clinical programs or services for multi-hospital systems strongly preferred.

Hours/Location:

  • M-F; Days
  • This position will work in a hybrid capacity with no set onsite cadence. The position will work from the HealthPartners Bloomington corporate headquarters a couple of times per week, or more as necessary.

Responsibilities

  • (45%) Program Development - Design and implement value-based care clinical programs that align with the organizational strategies and goals to drive success in value-based care. Oversee the development, implementation, and evaluation of clinical programs designed to enhance patient care and outcomes in a value-based care environment.
    • Data Analysis: Utilize data analytics to identify intervention opportunities, assess program effectiveness, identify trends, and drive decision-making. Collaborate with data teams to ensure the integrity and relevance of data sources.
    • Innovation: Foster a culture of innovation by identifying and implementing new technologies, tools, and processes that enhance care delivery and patient engagement that drive value-based care outcome results. Lead efforts to refine and enhance value-based care strategies based on feedback, outcomes, and changing industry standards.

  • (25%) Stakeholder Collaboration, Training & Education
    • Build collaborative partnerships and lead cross functional teams to execute value-based care tactics and initiatives across they health system.
    • Build trusting relationships with hospital leaders to successfully lead and implement new programs/processes.
    • Develop training materials and conduct workshops for clinical teams and leaders to enhance understanding of value-based care principles and practices.
    • Build strong patient and staff engagement strategies for understanding how to navigate value-based care resources and services appropriately to produce optimal outcomes.
    • Maintain focus on the desired mission, vision, and values while effective to ensure compliance to regulatory, accreditation and patient requirements.
    • Lead discussions with senior leadership on recommendations regarding policy development and/or changes.

  • (25%) Performance Monitoring - improvement, action planning and standardization to meet organizational goals.
    • Establish metrics and dashboards to monitor program effectiveness and impact of initiatives.
    • Evaluate and implement spread and sustainability plans.
    • Apply organizational and department values and continuous use of quality improvement principles in relationships, daily work, and program oversight.
    • Creates data priorities and report formats that display critical performance and outcome factors.
    • On an annual basis develops measurable, reasonable, and system-oriented goals for value-based care initiatives using tools such as driver diagrams.
    • Provides data regarding variances from guidelines, identifies, and leads opportunities for improvement.

  • (5%) Leadership/Other Duties
    • Participate in various committees, task forces, projects, and quality improvement teams, as needed and assigned.
    • Perform other duties as assigned.
