Position Overview

As a Home Health LVN, you’ll provide skilled nursing care under the supervision of a Registered Nurse (RN). You’ll ensure patients receive personalized care in the comfort of their homes, helping them achieve optimal health and independence.

Key Responsibilities

  • Deliver nursing care, including medication administration, wound care, and treatments, as directed by the care plan.
  • Monitor patient conditions and report changes to the RN and care team.
  • Educate patients and families on self-care techniques and wellness strategies.
  • Document clinical notes and patient progress accurately and timely.
  • Assist in coordinating care with the interdisciplinary team and physicians.
  • Adhere to all home health policies and regulatory standards.
  • Provide on-call support for patients as needed.

Qualifications

  • Education: Graduate of an accredited vocational nursing program.
  • Licensure: Active LVN license in the state and a valid driver’s license.
  • Experience: Minimum of 1 year in a clinical care setting, home health experience preferred.
  • Skills:
    • Strong clinical and problem-solving skills.
    • Excellent communication and interpersonal abilities.
    • Proficiency in documenting care and maintaining accurate records.
    • Current CPR certification.
  • Transportation: Reliable vehicle with valid auto liability insurance.

Work Environment

  • Provide care in patients’ homes with a flexible schedule.
  • Travel locally to serve patients in your community.
  • Physical activity includes prolonged standing, walking, and lifting up to 50 lbs.
