Position Overview
As a Home Health LVN, you’ll provide skilled nursing care under the supervision of a Registered Nurse (RN). You’ll ensure patients receive personalized care in the comfort of their homes, helping them achieve optimal health and independence.
Key Responsibilities
- Deliver nursing care, including medication administration, wound care, and treatments, as directed by the care plan.
- Monitor patient conditions and report changes to the RN and care team.
- Educate patients and families on self-care techniques and wellness strategies.
- Document clinical notes and patient progress accurately and timely.
- Assist in coordinating care with the interdisciplinary team and physicians.
- Adhere to all home health policies and regulatory standards.
- Provide on-call support for patients as needed.
Qualifications
- Education: Graduate of an accredited vocational nursing program.
- Licensure: Active LVN license in the state and a valid driver’s license.
- Experience: Minimum of 1 year in a clinical care setting, home health experience preferred.
- Skills:
- Strong clinical and problem-solving skills.
- Excellent communication and interpersonal abilities.
- Proficiency in documenting care and maintaining accurate records.
- Current CPR certification.
- Transportation: Reliable vehicle with valid auto liability insurance.
Work Environment
- Provide care in patients’ homes with a flexible schedule.
- Travel locally to serve patients in your community.
- Physical activity includes prolonged standing, walking, and lifting up to 50 lbs.
