Reporting directly to the Chief Health Initiatives Officer, this role will be responsible for leading a team of Community Health Workers (CHWs) to improve access to social and medical care and better health outcomes for individuals and families. This role focuses on addressing community health disparities through effective team management, training, and performance evaluation. The Supervisor ensures CHWs are actively engaging with community members, providing outreach, education, and support. Additional responsibilities include mentoring staff, collaborating with health systems, community partners, and stakeholders to align efforts with the goals of community health initiatives.

Position Summary:

The Foundation for Black Women’s Wellness (FFBWW) is seeking a full-time Community Health Worker Supervisor to help advance our commitment to Empower a Generation of Well Black Women. This position will play a key role in realizing and sustaining our organization’s mission to energize, mobilize and support Black women to transform their lives through education, advocacy, leadership development, and powerful partnerships.

Duties/Responsibilities:

Team Leadership and Supervision (40%)

  • Provide day-to-day supervision, guidance, and support to a team of CHWs, ensuring alignment with organizational goals and priorities.
  • Monitor and evaluate CHWs' performance through regular one-on-one check-ins, case reviews, and feedback sessions.
  • Set clear expectations for CHW roles, responsibilities, and performance standards.
  • Develop work schedules, assign tasks, and manage team workload to ensure equitable distribution and efficiency.
  • Serve as the primary point of contact for CHW-related issues, addressing challenges and resolving conflicts.

Training, Mentoring, and Professional Development (20%)

  • Conduct onboarding and training for new CHWs, ensuring a thorough understanding of organizational policies, and systems required to carry out and document work including but not limited to Apricot360 and Google Suite.
  • Provide ongoing coaching and mentorship to enhance CHWs’ skills in client engagement, documentation, and resource navigation.
  • Facilitate regular training sessions to ensure the team stays updated on best practices, community resources, and program changes.
  • Identify professional development opportunities for CHWs, including workshops, certifications, and continuing education.

Program Oversight and Coordination (15%)

  • Collaborate with the Chief Health Initiatives Officer to align CHW activities with program goals and objectives.
  • Monitor the team’s progress on key performance indicators (KPIs), including client outcomes and documentation accuracy.
  • Develop and refine workflows, processes, and tools to enhance the efficiency and effectiveness of CHW services.
  • Ensure compliance with all organizational policies, HIPAA regulations, and funding requirements.

Documentation and Reporting (10%)

  • Review and approve CHW documentation in Apricot360 or other systems for accuracy, completeness, and timeliness.
  • Compile data on team activities and outcomes, preparing reports for internal and external stakeholders.
  • Analyze documentation trends to identify areas for improvement or additional training needs.

Community and Stakeholder Engagement (10%)

  • Build and maintain positive working relationships with community organizations, healthcare systems, and other stakeholders to support CHW initiatives.
  • Represent the CHW team at community meetings, coalitions, and events to advocate for the needs of Black women and their families.
  • Work collaboratively with community partners to strengthen referral pathways and resource availability for clients.

Internal Collaboration and Organizational Support (5%)

  • Participate in FFBWW leadership and program team meetings to ensure alignment with organizational initiatives.
  • Provide input on program design, quality improvement initiatives, and strategic planning efforts.
  • Serve as a liaison between CHWs and organizational leadership, communicating team needs, successes, and challenges.

Required Skills/Abilities:

  • Ability to thrive in a collaborative, fast-paced environment with frequent changing priorities.
  • Tech Savvy with a firm grasp of MS Office, Google Business Suite, and various business and project management software.
  • Exceptional organizational, time management, and problem-solving skills.
  • A strong team player who is also a self-starter with a desire to take on new challenges.
  • Must be flexible and able to work outside of normal business hours.
  • Excellent organizational and leadership abilities, impeccable communication and people skills.

Education & Experience:

  • Bachelor’s degree in public health, social work, healthcare management, human services, or a related field.
  • Certification as a Community Health Worker (CHW) or similar credential, preferred but not required.
  • Minimum of 3–5 years of experience as a Community Health Worker or in a related community-based role.
  • At least 1–2 years of experience in a supervisory or leadership role.
  • Demonstrated experience working with diverse populations, focusing on health equity and culturally responsive care.

Compensation & Benefits:

  • Salary range: $58,000 - $65,000 annually.
  • Comprehensive benefits package including employer-sponsored health insurance, a Health Reimbursement Arrangement (HRA), a 401k plan with employer matching contributions, and optional life and disability insurance coverage.
  • 160 hours of Paid Time Off (PTO) annually, with 12 paid holidays each year.

Work Location:

  • This position will be based out of our headquarters in Madison, WI 53719, with a hybrid work arrangement allowing the flexibility to work 2 days a week from home.

Physical Requirements:

  • Prolonged periods sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
