Care Coordinator

Hours: Monday – Friday; 8am – 4:30pm, every 4th weekend

Salary: $77,937- $93,059

Job description:

As a Care Coordinator at Samaritan Bethany, you will:

• Manage care of residents based on the resident-directed care plan
• Provide professional communication with residents’ interdisciplinary team
• Comprehensively assess, report changes, and track ongoing resident clinical needs
• Supervise direct care staff by providing direction, completing performance reviews and disciplinary action
• Make changes to direct care staff schedule to ensure safe resident care

Requirements:

• Must have a MN RN License and be in good standing
• Prior long-term care experience preferred
• CPR Certification
