Case Manager - Care Coordination
Memorial Hospital at Gulfport
Coordinates the efforts of patient, family and healthcare professionals to develop a plan of care across the continuum based on identified needs. Reviews and assesses the appropriateness of patient admissions and continued stays in accordance with established policies and procedures.
Required Qualifications:
- Education: Graduate of an accredited program in Nursing with a minimum of an Associate’s degree.
- Licensure: Current MS RN licensure, compact license or temporary permit for same (valid for 90 days only).
- Experience: Three (3) years of clinical experience as a registered nurse in an acute care facility.
- Skills, Knowledge, Abilities: Expert clinical knowledge regarding disease management. Knowledge of performance improvement tools and techniques. Proficiency with computer applications including database management and statistical applications. Excellent customer service and communication skills; strong organizational and problem-solving skills.
Preferred Qualifications:
- Education: Bachelor’s degree in Nursing.
- Licensure: CCM, CPUR, CPUM or related certification.
- Experience: Experience in advanced quality management, utilization management, case management and/or DRG documentation compliance principles.
- Skills, Knowledge, Abilities: Advanced proficiency in resource / quality management methods, DRG coding, and third party payer requirements. Advanced statistical and spreadsheet / database application skills.
