Responsibilities
The RN Case Manager for Infectious Disease will coordinate medical and behavioral health services for patients in the Harm Reduction Program, focusing on those with HIV, TB, Hepatitis B and C, STIs, and syringe services. Key responsibilities include:
The RN Case Manager for Infectious Disease will coordinate medical and behavioral health services for patients in the Harm Reduction Program, focusing on those with HIV, TB, Hepatitis B and C, STIs, and syringe services. Key responsibilities include:
- Contributing to chronic illness care networks.
- Managing day-to-day and long-term patient care plans.
- Addressing health education needs of patients and families.
- Supporting chronic care patients in achieving self-management goals.
- Graduation from an accredited nursing program.
- Current Massachusetts RN licensure.
- Preferred case management experience.
- Bilingual skills in Spanish/Khmer/Russian are beneficial.
- Ability to work collaboratively in a multi-disciplinary team.
