Family Planning Nurse Practitioner

Job Summary:

The Community Health Center of Buffalo is seeking an experienced Family Planning Nurse Practitioner to provide outpatient care to patients in a modern facility. Responsibilities:

• Adhere to all clinical policies and procedures, including infection control and confidentiality.
• Assess physical and psycho-social health status through medical history and examination.
• Order and interpret laboratory tests.
• Triage, diagnose, and treat illnesses and physical conditions within the specialty.
• Perform therapeutic measures, prescribe medication, and provide preventive health maintenance and education.
• Teach, counsel, and educate patients, families, and groups.
• Participate in quality improvement activities.
• Comply with collaborating physician guidelines and regulatory agency requirements.
• Perform special assignments as needed.
• Prepare documentation, maintain records, and ensure information protection.
• Fulfill other relevant duties as assigned.

Minimum Qualifications:

• Licensed NYS, board-certified, Registered Professional Nurse Practitioner.
• Experience as a Family Planning Provider, including Long-Acting Reversible Contraceptive.
• Computer literacy with proficiency in MS Word, Excel, and Outlook. Experience with eClinical Works is preferred.

Salary Range: $103,000 - $108,000
