About We Care Daily Clinics:

We Care Daily Clinics (WCDC) is a warm, welcoming, and modern facility that provides treatment for addiction in a person-centered environment located in Auburn. WCDC seeks to renew hope and purpose for those battling opioid use by removing barriers, providing a holistic approach to recovery, and utilizing evidence-based treatment to restore healthy living. Our top priority is to provide the highest levels of patient-centered care to those we serve.

WCDC is an Opioid Treatment Program (OTP) facility using FDA approved medication paired with behavioral health counseling. Our individual service plans are built around patients in a supportive, safe environment to create a personalized pathway to recovery. In addition to recovery, we also offer planned patient transportation and family and social support.

WCDC's first two mobile clinics launched October 2022 in Seattle and Tacoma. The mobile clinics offer the same evidence-based, quality treatment that is available at the Auburn clinic, including medication for opioid use disorder, behavioral health counseling, peer support, transportation, telehealth, and referral services. A third mobile unit will be launched later this year in the SODO district.

Job Summary:

Supporting Patient Care and Engagement

The Care Navigator plays a pivotal role in patient care by offering on-demand peer services, crisis intervention, and de-escalation support. Stationed in the lobby, the Care Navigator is a vital resource for patient engagement, pre-registration assistance, and overall care coordination. They serve as a bridge between patients and healthcare providers, ensuring seamless access to services and support throughout their care journey.

Responsibilities:

  • Patient Engagement: Assist patients with the Web-App and other pre-registration tasks, ensuring they receive the support needed to navigate their healthcare journey.
  • Crisis Intervention: Provide immediate support for mental health crises, including suicidal or homicidal ideation, by employing motivational interviewing and de-escalation techniques.
  • Queue Management: Monitor patient queues, ensuring patients are attended to promptly by alerting the SUD team if a patient has been waiting longer than 5 minutes.
  • Crisis Support: Offer emotional and de-escalation support across departments, helping patients navigate overwhelming feelings and barriers to care.
  • Administrative Support: Manage critical reports, including the 45/90-day report and UA intervention report, while supporting SUD teams with rooming and patient care as needed.
  • Departmental Coverage: Provide backup for the PSR desk and the Neighborhood Center, assisting in answering questions about the SUD/Peer program and supporting overall clinic operations.
  • Additional Duties:
    • Assist in arranging patient transportation.
    • Generate billable check-ins for peer services.
    • Advocate for patients, helping them overcome barriers to their care.
    • Ensure smooth patient transitions, especially for unenrolled and outreach patients.
Education & Experience:

  • High school diploma or GED equivalent
  • 2 years customer service experience
  • Basic knowledge of computers and programs, including MS Office, PowerPoint, and Outlook
  • Preferred: AA degree, CPC or int he process of obtaining a CPC
  • Opioid Treatment Program experience will be given hiring preference.
