Job Summary:

The Specialty Care Coordinator (Social Worker) plays a crucial role in advocating for and coordinating care for patients with complex medical needs. They work closely with healthcare providers to ensure that patients receive the specialized care and support they require to manage their conditions effectively.

Responsibilities and Duties:

• Conduct comprehensive assessments of patients' medical, social, and emotional needs
• Develop individualized care plans in collaboration with patients, families, and healthcare team members
• Provide counseling and support to patients and families to help them cope with their health challenges
• Coordinate referrals to specialty providers and community resources

Qualifications and Skills:

• Master's degree in social work or related field
