The Neighbor Network of Northern Nevada (N4) is seeking a caring and motivated person to join our team as a Northeastern Nevada Community Care Program Coordinator. Do you love your community? This Community Care Program Coordinator covers Northeastern Nevada and oversees a team of Community Care Partners providing respite and supportive care. Community Care Program Coordinators are charged with program monitoring and information tracking as well as oversight to support N4’s grant reporting and contracts. This position will implement strategies which support state Regional Center requirements and N4’s mission; follow regional center policies and procedures; and align with person centered values and principles for self-determination.
Responsibilities
Community Care Program Coordinators are responsible for:
- Completing intake tasks including conducting new member orientations and related paperwork
- Drafting ongoing quarterly and annual reports for services provided in N4’s programs
- Assisting with staff training and oversight
- Engaging in home visits and telephone and email communication with N4 members and partners
- Scheduling and tracking services
- Obtaining feedback from members and other stakeholders
- Keeping various program data organized and assisting with grant reporting and related fiscal activities
- Assisting the N4 team with the ongoing implementation of new programs
- Filling in support sessions as needed
- Attending special events and activities as assigned
- Travel to rural areas of Nevada as required
Qualifications/Requirements
- Bachelor’s degree from an accredited 4-year college or university in a health or human services field required
- Master’s degree from an accredited college or university in a health or human services field preferred
- Minimum of one year of experience working directly with people with intellectual disability or other development disabilities and/or older adults
- Excellent written and verbal communication skills
- At least 3 years of supervisory experience preferred
- Demonstrate good problem-solving skills and maintain patience with people served
- Ability to adapt to service program changes
- Maintain reliable transportation
- Keyboard and computer skills to utilize word processing, spreadsheets, and databases for communications and record keeping
- Must be reliable and willing to work in-person in the office and community (no remote work)
- Must be willing to travel as needed through Northern Nevada (up to 20%)
- Must show proof of a COVID vaccination within the last 12 months, before start date
