Overview

The maternal mental health screening and support coordinator would provide resources and support to mothers and families through appropriate screening tools, support groups, referrals, and community resources.

Responsibilities

  • Identifies and coordinates support for families to help find resources for perinatal mental health which includes therapists, doulas, groups, and family resource centers.
  • Assists in the development, implementation, and maintenance of structures and processes which support social work, pregnant and post-partum women for clinical and non-clinical improvement efforts.
  • Ensure maternal and mental health screenings are done at appropriate intervals in pediatric visits during pregnancy and postpartum per screening guidelines to improve patient care outcomes.
  • Provide care coordination support for families experiencing difficulties getting needs met: to include optimal patient coordination, excellent collaboration between teams (OB/GYN, primary care, and pediatrics) with a focus on family experience and outcomes.
  • Provide ongoing staff education (nurses, support staff, providers) in the CGP that work with perinatal women about the importance of perinatal mental health, appropriate screenings, workflow process, follow ups, coordination of care, culturally sensitive care, compassion fatigue, and other relevant topics.
  • Provide education for perinatal mental health disorders to patient and family including sign/symptoms of perinatal mood disorder, what to do if patient is in crisis.
  • Support linkages between patients, the health care system and social services resources.
  • Identifies patients/families appropriate for Care Coordination services in a systematic manner
  • Provide group and 1:1 support to include peer support groups.
  • Participate as part of a multi-disciplinary team providing maternal mental health screening response.
  • Complete documentation of all relevant conversations, meetings, and referrals.
  • Note and escalate patient concerns or needs to the appropriate staff as appropriate.
  • Participate in the updated trainings related to identifying the best maternal mental health screening and response tools and protocols for optimal patient experience and outcomes.
  • Demonstrate collaborative and respectful behavior.
