We are seeking a full-time Registered Nurse Case Manager for our mission-driven hospice and palliative care organization, recently recognized as a Top Place to Work in Massachusetts. This role involves local travel to provide care in patients' homes and residential facilities in Greater Boston and parts of Metrowest. A $5000 sign-on bonus is offered.

- Develop and manage comprehensive care plans in collaboration with the interdisciplinary team.
- Conduct clinical assessments and monitor patient care for symptom relief.
- Educate patients, families, and healthcare professionals about hospice care and end-of-life processes.
- Licensed Registered Nurse in Massachusetts.
- Understanding of end-of-life issues; experience in hospice or related fields preferred.
- Strong assessment, organizational, and communication skills.
