Position Overview:

The Program Manager of Agency Support Brokerage manages the Agency Support Brokerage department, ensuring quality service delivery. This hybrid position combines remote and in-person work, with specific office days based on team needs.

Key Responsibilities:

  • Supervision and Management: Oversee the Agency Support Brokerage team, facilitate meetings, manage performance, and handle day-to-day operations.
  • Case Management: Oversee a small caseload of individuals in the program, ensuring health and safety and program satisfaction.
  • Community Engagement: Build relationships with community partners, assist with recruitment and enroll individuals in programs as needed.
  • Relationship Development: Maintain regular contact with supported individuals and families to ensure program satisfaction.
  • Interdisciplinary Collaboration: Participate in support meetings and collaborate with other program managers on system updates.
  • Intake: Support the intake process and develop initial Self Direction plans in line with person-centered standards.
  • Compliance: Ensure adherence to OPWDD regulations and agency policies, including training on relevant regulations.
  • Case Management Oversight: Maintain accurate documentation and communication regarding services provided.
  • Advocacy: Advocate for the rights and needs of supported individuals, facilitating access to necessary resources.
  • Reporting: Ensure proper reporting and investigation of incidents as per regulatory requirements.
  • Professional Development: Attend trainings and seek development opportunities to further professional goals.
  • Perform other duties as required.

Qualifications

  • Age Requirement: Minimum age of 18.
  • Experience: 2+ years in OPWDD HCBS Waiver programs required; supervisory experience preferred.
  • Educational Qualification: Bachelor’s degree in a related field preferred.
  • Certifications: Support Broker Authorization required.
  • Driver's Status: Valid License preferred; ability to travel required.
  • Physical Demands: Tasks may involve assisting with toileting and hygiene, with accommodations available for individuals with disabilities.
